Pressure injuries can develop due to immobility, shear, and moisture. To effectively assess and create a care plan, the National Pressure Injury Advisory Panel (NPIAP) has established a staging classification system. This system helps determine the extent of damage to skin, tissue, and structures through visible and palpable wound assessments.

Moisture-associated skin damage (MASD) is a frequent cause of skin breakdown but is largely preventable. Effective prevention involves a comprehensive patient assessment, selecting the appropriate products, and ensuring their correct implementation.
