Description & Objectives:

A skin assessment is an essential component to help gain insight into your patient’s overall well-being! A head-to-toe assessment should be completed upon admission.

Additional skin assessments are necessary with changes in the patient’s health status and risk factors for skin breakdown and wounds. This will assist with the detection of skin changes, including moisture, texture, color, temperature, and swelling, that may indicate further investigation.

Upon completion of the program, the participant will be able to:

State the time frame for an initial skin assessment and when follow-up skin assessments should be performed.
Describe the location and appearance of 3 different skin conditions that may be observed during a skin assessment.
List 3 factors that can cause skin breakdown.
